Organizations rarely fail to improve because they lack tools. Something happens between the solution and the system.
Who we are
Conscious Systems Design is an independent applied research organization investigating the human conditions that influence how people and organizations observe, learn, adapt, and sustain improvement.
What we do
Research in the Real World
Conscious Systems Design partners with organizations, researchers, and regional networks to study human systems where work actually happens. We explore persistent challenges through observation, small-scale experimentation, and applied research - connecting real organizational questions with opportunities to learn what actually works.
What we believe
The Future of Operational Excellence is Human.
Founder
Angela Wolfrum
Founder & Systems Architect
Conscious Systems Design was founded by Angela Wolfrum, an operational excellence practitioner and researcher whose work spans more than two decades of organizational improvement, systems thinking, and human-centered change.
Her work explores a recurring question: Why do technically sound solutions succeed in some human systems and fail to take hold in others?
Drawing on that experience across complex industrial and organizational environments, Angela founded CSD to investigate these questions through observation, experimentation, interdisciplinary research, and collaboration between practitioners and academic researchers.
Why do good solutions fail to stick?
Organizations invest enormous effort in improvement. Processes are redesigned. People are trained. Systems are implemented. Results improve.
And sometimes, months later, the organization quietly returns to where it started.
The technical solution may have been sound. So, what happened?
What We’re Exploring
What happens between a technically sound solution and the human system expected to sustain it?
Conscious Systems Design is exploring the conditions that influence whether organizations learn, adapt, and sustain improvement over time.
Our areas of inquiry include:
Relational Patterns:
How everyday interactions, roles, expectations, and responses shape what people do next.
Organizational Learning
How organizations notice, experiment, learn, and incorporate new knowledge into the way work happens.
Adaptive Leadership
How leadership behavior influences ownership, decision-making, psychological safety, and the capacity to respond to changing conditions.
Improvement Sustainment
Why technically successful changes sometimes become embedded - and why others slowly return to the previous condition.
Discernment & Observation
How the ability to observe without immediately fixing may help organizations distinguish symptoms from the systems producing them.
We are not beginning with an answer.
We are beginning with a question worth investigating.
Research
CSD studies the human systems that influence whether improvement takes hold, adapts, and endures.
Our work combines field observation, organizational experimentation, and collaborative research to examine what happens between a technically sound solution and the people and systems expected to sustain it.
Field Research
Studying improvement where work actually happens.
Applied Experimentation
Testing small changes in real organizational systems and observing what happens next.
Research Collaboration
Partnering with organizations, practitioners, and academic researchers to build knowledge across settings.
Human Standards Before Automation
As organizations rapidly integrate AI into work, CSD is investigating a foundational question: What human conditions should exist before we automate?
We are not beginning with an answer.
We are beginning with a question worth investigating.
Frameworks
CSD develops practical frameworks for noticing, understanding, and responding to complex human systems.
These frameworks are not intended to prescribe a single answer. They create structure for observation, experimentation, learning, and adaptation.
A framework should help us see.
Not tell us what to see.
OBSERVE
Discernment & Observation
Learning to remain with what is happening long enough to distinguish observation from interpretation - and symptoms from the systems producing them.
Emerging work: The Observer, B-DADI (Behavioral Awareness, Disciplined Inquiry, Adaptive Framing, Discernment Under Pressure, and Integrated Practice)
NAVIGATE
Adaptive Action
Moving from awareness to purposeful action while remaining responsive to what the system reveals.
Frameworks: PCA (Purpose, Clarity, Action), CALM (Clarify, Act, Learn, Modify), and 5As (Awareness, Acceptance, Alignment, Action, Adaptation)
LEARN
Organizational Learning
Creating visible cycles through which people notice conditions, test ideas, learn from results, and incorporate new knowledge into the way work happens.
Applied framework: OCN (Organizational Catalyst Network)
Speaking
CSD brings emerging research into conversation with the people doing the work.
Speaking engagements explore the human side of operational excellence: why technically sound solutions fail to sustain, how relational patterns influence what happens next, and what becomes possible when organizations learn to observe before they fix.
OPERATIONAL EXCELLENCE
The Human System Behind the Process
Why process improvement succeeds technically yet struggles to become embedded in the organization.
OBSERVATION & DISCERNMENT
The Observer
What changes when leaders and practitioners learn to notice more, conclude less, and become curious before acting.
RELATIONAL SYSTEMS
What Happens Next
How everyday interactions, expectations, roles, and responses influence whether people speak, decide, experiment, learn, and adapt.

Technical & Relational Systemic Problem Solving: Can AI Diagnose Dysfunction?
Angela Wolfrum & Susanna Watson
Human Systems & Relational Pattern Recognition
Interactive conference session exploring why technically sound improvements fail to sustain and how relational patterns might shape what happens next.
The conversation is not separate from the research.
It is a part of it.
Insights
Ideas emerging from observation, practice, and research.
CSD shares developing perspectives on human systems, organizational learning, discernment, and improvement.
These are not presented as finished answers. They are observations, questions, and ideas worth examining more closely.
Observe Without Fixing
What becomes visible when we resist the urge to solve too quickly?
When the Solution Isn’t the Problem
Why technically sound improvements can struggle inside the systems expected to sustain them.
The Nudge Before the Answer
What intuition may be telling us, and why a “gut feeling” might be an invitation to become curious rather than a reason to act.
We write to examine what we are noticing.
Not to declare what we know.
Let's Work Together
Some questions are worth investigating together.
CSD Collaborates with organizations, practitioners, and researchers interested in understanding the human systems that shape how improvement takes hold, adapts, and endures.
You may have a persistent challenge. You may be interested in research, experimentation, speaking, or collaboration. Or you may simply recognize something here that connects with what you are observing in your own work.
Tell us what you’re noticing. We can explore together what collaboration might look like.
You do not need to arrive with a solution. A good question is enough.
